   Required Documents

  1. Request Replacement of Passport Application Form
  2. New front view colored personal photo size 6x4 with a white background
  3. To modify profession details: Recent certificate from the employer stating the profession in both Arabic and English
  4. To add a fourth name: Copy of the Father’s or Brothers’ passport if the fourth name is provided, if not, the applicant should attend personally to sign the declaration
  5. To modify the name in English: Write the name in English on the Passport Replacement Form
