This service is provided by the Ministry of Health to Individuals requesting to open a new record for practicing businesses of consumer goods, such as perfumes, cosmetics and house disinfectant products.

   Required Documents

  1. Copy of the Commercial Registration
  2. Business license application form
  3. Copy of the Identity Card
  4. Lease agreement stating the address and provide the attachments as per the activity
  5. Authorization letter for the personnel authorized to conduct the procedures
  6. Architecture drawings for the establishment
  7. Copy of approval form from related stakeholders

Ministry of Health will submit the requirements of practicing the business to the applicant.
