Fitness and Sports Activities Examination Request

This service is provided by the Ministry of Health to Individuals requesting to receive a medical examination for fitness activities, for:

  1. Domestic workers (butler, cooker, gardener, driver and babysitter).
  2. Bahraini employees (administrators and worker).
  3. University Students.
  4. Athletes.

  2. 3 passport photos
  3. Fitness examination form

 

Employees:

  1. Identity Card
  2. 3 passport photos
  3. Medical examination request letter from the employer

 

For university students:

  1. Identity Card
  2. 3 passport photos
  3. Admission letter from the university

  • Exceptions:
For athletes, the form should be stamped by the doctor first
