Copy of the Commercial Registration
Copy of the Tourism License
Valid report issued by the General Directorate of Civil Defense
Last notification showing the schedule of site visit from Ministry of Health
Manager’s qualification, experience and Curriculum Vitae
Copy of the Manager’s Identity Card
Names of all local and foreign employees (designation and nationality) on stamped official letter head
Medical inspection of all kitchen workers(for restaurants and hotels)
Commitment letter in case of late renewal
