  1. The original marriage contract and a copy of thereof, or the equivalent of an external contract and a copy of thereof or the previous certificate of prove of marriage and a copy of thereof
  2. A copy of the Identity Card or the passport of the husband and wife
  3. A copy of the wife’s mortality (Death) certificate
  4. Copy of husband’s and wife’s Identity Card or passport
  5. Certificate of proof of the wife’s mortality (Death) application form
